Either complete multiple G-1450 Forms or request separate checks...the H-1B petition will be rejected if the fees are combined in one G-1450 or check.
If you are paying a fee with a G-1450, do not list the applicant's name on the form. Instead, list "Temple University" in the "Given Name" field and leave "Family Name" blank.
